Home Blood Pressure Log Submission

Use the form below to help you calculate your average blood pressure readings and submit directly to us :

Terminology:

  • Systolic – top reading
  • Diastolic – bottom reading

Instructions

Measure your blood pressure twice a day in the morning and late afternoon at about the same time each day if possible.

For best results, sit comfortably with both feet on the floor for at least two minutes before taking a measurement. 

When you measure your blood presssure, rest your arm on a table so the blood pressure is at the same level as your heart.

Send the results to your clinician by submitting the form.

Definitions & Targets

Average home or ambulatory blood pressure readings persistently above 135/85mmHg (home readings) is consistent with hypertension
BP readings in the clinic room can be a little bit higher which is normal, so levels persistently above 140/90mmHg is consistent with hypertension.

Hypertension if left untreated can be associated with cardiovascular disease i.e. heart disease, heart attacks, angina, strokes and kidney problems. 

Target BPs vary depending on underlying conditions and age but should aim for less than 140/90mmHg (clinic) or less than 135/85mmHg (home).
